Coffee significantly reduces absorption of levothyroxine (Synthroid, Eltroxin) when taken close together. Studies show coffee can reduce drug absorption by 30-50%.
The rule: take levothyroxine on empty stomach with plain water. Wait at least 60 minutes before coffee, food, or other beverages.
What blocks absorption:
- Coffee (especially with cream/milk)
- Tea
- Calcium-fortified juices
- Milk and dairy
- Iron supplements
- Calcium supplements
- High-fibre foods (bran cereals)
- Soy products
- Antacids and PPIs
Optimal routine:
1. Wake up
2. Take thyroid medication with plain water
3. Wait 60 minutes
4. Drink coffee, eat breakfast
Who is most affected: people on levothyroxine who feel tired despite taking medication. Often the issue is poor absorption, not dose.
If you cannot wait 60 minutes: take medication at bedtime instead โ 3+ hours after last meal. Studies show this is equally effective and avoids the morning timing issue.
Alternative thyroid drugs (Tirosint, NDT): less affected by food/coffee but still take 30 minutes apart for best absorption.
Monitor TSH levels regularly to ensure absorption is adequate. If TSH stays elevated despite increasing dose, absorption is the issue โ fix the timing.
