B12 deficiency develops slowly โ symptoms can take months or years to appear, but neurological damage can be permanent if untreated.
Early signs (first 6-12 months):
- Persistent fatigue not fixed by sleep
- Brain fog, difficulty concentrating
- Mild memory issues
- Slight tingling in hands or feet
- Glossitis (red, swollen tongue)
Late signs (12+ months):
- Pronounced numbness in extremities
- Balance problems
- Severe fatigue
- Mood changes, depression, irritability
- Pale skin
- Megaloblastic anemia
Who is at risk: vegetarians/vegans (B12 only in animal foods), elderly (absorption declines with age), anyone on PPI medications for over a year, anyone on metformin long-term, people with autoimmune conditions.
Get tested: serum B12 (basic) and methylmalonic acid (more sensitive). B12 below 300 pg/mL is suboptimal even if "normal." Below 200 is deficient.
Food sources: liver, sardines, beef, eggs, dairy. Vegans must supplement โ there is no reliable plant source.
