The complete answer

For probiotics, strain selection matters far more than exact timing. But timing does affect survival through stomach acid.

Best timing: 30 minutes before a meal or first thing in the morning on empty stomach. Stomach acid is lowest then, so more bacteria survive.

With food: only if specifically labeled to take with food, or if it causes nausea otherwise.

Strain matters most:
- IBS-D: Bifidobacterium infantis 35624 (Align brand)
- IBS-C: Bifidobacterium lactis B420
- After antibiotics: Saccharomyces boulardii
- General: multi-strain with billions CFU including L. acidophilus, B. lactis

Dose: most products have 10-50 billion CFU. More is not necessarily better.

Duration: minimum 4-8 weeks to see effects. Some people need ongoing.

Food-first approach: kefir, yoghurt, kimchi, sauerkraut provide diverse strains naturally. A daily serving of fermented food is often more effective than supplements.

Watch out: during SIBO (small intestinal bacterial overgrowth), probiotics can worsen symptoms. Get tested if probiotics consistently make you bloated.
