Beer is the worst alcoholic drink for gout โ far worse than wine or spirits. The Choi study (Lancet, 2004) found beer increased gout risk 49%, spirits 15%, and wine had no significant effect.
Why beer is worst: beer has high purine content from yeast, plus alcohol that impairs uric acid clearance. The combination is uniquely damaging.
What to do if you drink:
- Avoid beer entirely if gout-prone
- Wine: 1 glass dry red occasionally is generally safe
- Spirits: clear spirits with soda water (not tonic โ sugar) in moderation
- Always with plenty of water
The safest choice: no alcohol during active gout treatment, especially when starting medication. Once uric acid is controlled, occasional wine is reasonable.
Key rule: never have multiple drinks per day if gout-prone. The risk rises non-linearly.
